Generally it's when there's a disperancy between the artist ability to draw a realistic body and their ability to do anything else.

>Lines mysteriously falling perfectly into place with no sense of lineweight, form or construction. Terrible folds, crosshatching and when there's information missing suddenly they don't now how to draw, hence the misformed dick.
