Her facial features are misaligned.
All your edges are fucked up; your hard edges are undefined, and your soft edges are poorly blended.
The banding effect is basically preventing you from conveying form and mass.

You can try starting off with 100% opacity, and just 3 colors, or values rather: base value, shadow, and light. Then blend them together accordingly.

You show no understanding of value, since you're color picking from the reference (maybe even at 1pixel precision even) without fundamental knowledge (or use of them anyhow), and thus the values are all over the place.

You show no understanding of form either, since you're just splotching on top of a ref, and when you see the hue differ too much you pick a new color.

You don't blend colors, so there's no edge variety to help give a sense of shape and form. Just hard edges laid down with ill considered strokes.

You should post the reference you're using, at least. But don't kid yourself. It's your own time and energy, you know? There's not an infinite amount of those.
