actually a good question, because everyone's anatomy is slightly unique, you will personally have to find what feels the most ergonomic to you. try out suggestions and do some trial and error.

some people do hover, mostly you'll find hovering in overhand style.
many don't hover when drawing in the normal style, thus the reason for smudge guards
