And some custom brushes. You can try to paint some brush profiles by hand and have fun experimenting (pic related). Photoshop's default textures are pretty boring though.

You're right, I wanted the face to be visible, that's why I choose this angle. I haven't though of the arm position you drew, I'll consider it.
