you can give the truman a shot and just return them if they don't feel right, but i think you'll run into the same issue as me desu. your toe splay looks almost identical to mine. the length and ball width was perfect by going down half a size from brannock on length and true to width, but the rubbing on the big toe was ultimately a deal breaker. i unfortunately didn't figure it out until i tried to work in them and realized every time i did anything besides normal walking it was rubbing on the knuckle of my big toe.

I bought Wesco Mr. Lous off the rack, and they're without the slightest hint of irony the best-fitting boots I've ever owned.
The best-fitting leather shoes I've ever owned, period.
I have a size 13-14 US (46-47 EU) and apparently my feet are pretty wide at the front of the foot. I bought size 11.5 E Wesco Mr. Lous because that was the biggest they had, and they fit like no other boot I ever owned.

Before the Wescos, since boots in Europe tend to have narrow lasts and pointy toes, I always had to either:
1) deal with the fact that the front of my foot and toes are getting squeezed from the sides
or
2) buy a larger size for the extra width; which obviously fucks up the look and rest of the fit, and I could never find those sizes anyway.

Not so with the Wescos.
I don't have any experience with other makers, but it's almost impossible to imagine a better fit.
With some thicker socks they fit like gloves, but are a little harder to get off.

I can't really find a difference between lets say the Darn Though Hiker Boot Midweight with Cushion and Tactical Boot Midweight with Cushion, except for the colors. don't go for leather soles on PNW boots
leather soles wear out really fast and need frequent resoles, stitchdown/handsewn stitchdown doesn't lend itself well for frequent resoles so you want a long wearing sole on them, any of the vibram rubber soles should be fine
you can add toppies aka sole savers on leather soles by a cobbler and have that wear out instead of your leather soles, but it's a hassle

also if you don't really know what the options on custom boots mean or what the purpose is, it's best to skip over them
i know it's tempting to go all out with options, when you get the opportunity, but it's better to stay simple, because you can ruin a boot without knowing

Yeah, I’m not going to assert that the Wescos are inherently bad boots by any means, but they just were not the best last for my feet. Since I live pretty close to Scappoose, Oregon I even went to the Wesco factory to get sized perfectly. After purchasing a pair of Mister Lou’s from Bakers and wearing them in for 6 months, they never really became comfortable. The most uncomfortable part of the boot was the heel counter. It just has very little shape and volume, and it is in fact made with thermoplastic material. Apparently they never used to do that with their counters, but I guess they’ve more recently been using inferior materials that way. I really wish they fit me because I like the Wesco look and I like how robust and tanky they are, but when I was still getting aches and blisters on my heels after completely breaking them in, I had to just sell them

What does that mean?

In my experience, all commando and even ridgeway soles (including OG ridgeway and Carmina 'ridgeway') are clickety clackety EXCEPT Carmina's "country" soles which are made from hevea milkers.
These soles are dead silent, even more than sneakers. But you can squeeze them with your finger, I'm not sure they will last very long.

All low profile soles like dainite, Carmina's 'not dainite' and TLB's City are very clickety clackety.
I have no shoes with leather soles but I do have loafers with leather sole + topy. Also very clackety.
