I am having the same dilemma as you after owning some painful shoes I am now hyper-aware of how my shoes crush my toes. My solution is to either size up so there is extra length AND width, or just search for a better fitting shoes. But the pointyness of the shoes means that you need to have extra length for everyone. Still, some shoes will never be comfortable and sizing up can never fix it because they become too loose around the ankle or simply way too long.

I bought some NBs and they feel uncomfy (not painful) with my orthopedic soles. The length & width of the NBs are good, just the height is not enough I guess.
Will they adjust to my feet a bit and get comfier? I really don't want to return the NBs because I won't find the same model again, and I don't want to look for another shoe.

I believe the only sneaker company that does this is NB, but the differing widths are only available on certain models.

Traditional leather shoe companies generally have multiple width available. Sneakers, being the elastic, one-size-pretty-much-fits-all cushion, don't generally warrant multiple widths.

If you are in that group that has exceptionally wide feet, then your shoe options in general are limited in comparison.

My experience completely. I'm a 13 in sneakers, so I got black CP lows in 45, based on everybody's advice to size down. When I finally wear them, they're too tight and too long, so my toes were being strangled and the heel kept slipping and rubbing against my ankle. If I got a 46, the heel slippage would have been even worse.

I guess CPs are not right for me.

Other than the aforementioned CPs, I haven't bought any expensive sneakers. I have a few pairs of boots (Alden, Red Wings, Wolverine) that I sized down a full size and wear regularly, but that's about it. If I ever shell out hundreds of dollars for designer shoes again, I'll make sure that they aren't narrow or just go true to size.
