My mum had trich as well, and honestly anxiety medication was the only thing that helped her. I struggle with dermatillomania but it's not nearly as bad as her issues were. Being hyper conscious of it and forcing myself to stay distracted helps, but I still have bad days where I pick constantly. You're hair is actually really nice, if you put a lot of effort into caring for it you will be less likely to pull it out. Dont fuck with your ears man, nobody notices it I promise you. I probably had the biggest case of elephant ears I've ever seen where they stuck out at an almost horizontal angle, it was a big insecurity. However, nobody really thought anything of it because ears are invisible to most people. I ended up getting them pinned back and there were a lot of after effects that still mess with me to this day so I'd honestly recommend staying away from anything involving ear surgery

I actually recently cut off about 6 inches—it was way too much. If I were to cut it I think I'd go for something short as I haven't had short hair in ~15 years and I'm just curious (but like I said, worried I'd never be able to grow it back to this). My ears aren't as flat as I'd like them to be (but nothing crazy), which is why I started wearing long hair in the first place.

Re: your chin scar—I'd say don't do anything about it. I've never looked at chin scars as aesthetically displeasing. Almost, weirdly, gives some character without detracting from your overall look. So yeah, I'd save leave it...but also I have no idea how you'd heal a scar.
