It's not that bad. The first 10-15 minutes are the toughest, but then you settle in to it. It's done in small stages anyway, so the artist will do a line, wipe it, get some more ink and do another line. You don't constantly have the machine on you.

Any of the above. You message the artist, or ring the studio and say you want to book in with the artist. If you haven't picked an artist you can message the shop the idea directly and they'll pick someone (usually whoever works in the style you want).
Read their instagram or facebook and see if they have a preferred e-mail for bookings.

Email or facebook is best because it allows you to send the reference images easily. Send as many reference images as you can get your hands on. More is better if possible. Obviously don't be sending like, 20 images, but for this one I sent 8 images >>12796354. Also send a shot of the area you want it in and try to give an idea of the size. If you haven't decided that either you can ask the artist.
