As for the undershirt, not too fond of the pattern. Just get a nice plain white button up- solid colours are best. That being said, you do have this mismatch in style. Alright OP I'll help you. First thing you should do is go to a more fashion centric store, H&M, uniqlo, or maybe forever 21 (depending on your budget you could also get Levi's 511 or 513.) Grab a bunch of jeans and try them on in the dressing room to find ones that fit you the best, you'll want to go for something slimmer on the ankle for a much more flattering look. You don't have to go full skin tight skinny just something slim. Browse /fa/ or even /MFA/ for Inspo on how you want your jeans to look. You should try to find the perfect length or a little more if you want to cuff and not let it sag as much around the ankle and cause stacking. Look at pic related, while the jeans don't fit perfectly around her ankle, it still looks good from the lack of all the little creases in the jeans.

As for your sweater, crewnecks are a lot easier to style than v necks. I personally don't really like them and they're more suitable for a formal setting, they don't go well with faded denim and converse. Crewnecks can go with almost anything and look a lot more chill, v necks makes you look like a try hard.

The checkered shirt isn't good either, go for a more broad flannel with faded colors like red and green. I also would say you shouldn't do plaid under sweaters, you should stick to a solid color like black or white, whichever one goes better with the color of the sweater. But this is just my personal style.
