Ramones typical fit true to size. If you're a 42 in CP you'll be able to do a 42 in Ramones highs because of the zipper.

The lows are very hard to get into I would even size up a half a size (get at least a 42.5 or a 43 if you have high arches).

I wouldn't fully replace. A cobbler should be able to carve in some rubber caps that will look nice. I recommend getting heel taps though, every pair of sawtooth Ricks I've owned have suffer pretty gnarly from it and I don't drag me feet at all. I think this is why the drkshdw sneakers have a flat rubber panel at the heel and toe now.

Those are old geos, they have a crepe sole which is a relatively simple for cobblers to replace with a piece of crepe rubber. With the sharktooth ones, I think it may be more difficult to just slap a piece of rubber over them.

Those are dunks. Not geos.

With sawtooth soles that geos have, a cobbler will typically shaved down the area where they are working on an shape some rubber to fit. Typically this is for heel drag. If the teeth are very worn down they will likely want to smooth out the sole and cap with a crepe sole.
