I will admit that I have not studied the phenomenon as extensively as you appear to have, but I have another theory.

It may be that Ezra's feet are uncommonly large, however I do not believe he has any trouble finding his size, nor does he have to deal with squeezed toes. I present Exhibit A: pic related. These are women's shoes, and clearly too large at that. So, that there are shoes large enough to fit Ezra's supposedly gargantuan feet is almost assured. Even if they are scarce, we must remember that he is rich. This would not be a problem for him.

Now, let us move on to the real reason for Ezra's fucked up feet: his penchant for the very high heels he loves so much, compounded by the fact that he is not exactly a slight little waif. All that pressure on his toes was bound to disfigure them.
