>There are times I know exactly what I want, but I just can't find it in designer fashion or elsewhere.
>Has anyone else felt that?
Absolutely. I'm surprised you're new to fashion. The fact that you know what you're looking for says a lot of good things about your current mentality. Shows you have vision, you see clearly how you want to look.
>And what have you done about it?
Honestly I usually just wait. This works out for me because the garments I imagine are usually pretty simple.
I've wanted a dark green parka for years and none I found matched the fit and detailing I imagined but I just kept looking. Eventually I found the Fjallraven Greenland Parka and knew it was exactly what I was looking for.
Same thing has happened with a few other articles I own.

>Do you have your clothes modified by tailors you trust?
I haven't but I definitely would. I've considered some cut and sew options for a few sweatshirts/jackets I own.

>Do you even create new clothes with the help of such tailors?
Getting this done at a laundromat/tailor might not work out for you, but I'm sure there are a few more focused tailors around your area. Here we have a small tux rental and retail shop that does a pretty good job.
>And how do you exactly do that?
^

Chances are if you go back enough you'll eventually get to bro out with your tailor about the dope sleeves you attached to your bomber or w/e. If they've made clothing their career they'll probably appreciate your thoughtful approach to it and you'll have something to talk about.
