You need a proxy site to place bids unless you live in Japan. I use Jauce, it allows bidding in real time, but some say the fares are relatively high - I have friends who speak highly of SutoCorp. It'll ship to their facility, and then you have it shipped to you - it's good if you win lots of auctions and have multiple items arrive at their warehouse when it is then sent to you, as it saves on shipping.
Yes and no - there are some auctions you bid and some auctions you can Buy It Now. BIN auctions work just like eBay, whereas the actual auctions work like an auction - the length of the auction can change if there is still bidding going on, and it can get quite competitive. I've know auctions to go on for over 2 hours over the time it was supposed to end.
