Try to have to have a mature, honest, and non-emotional conversation with her about it. Let her know you're concerned about her health and that it's also affecting you in the bed room. Keep it mainly focused on health though and make it so that having a new diet and exercise routine will be a lifestyle change for the both of you. It's easier if she's got a partner participating in the weight lose.

Even with that she'll most likely still feel offended, there's nothing you can do about that, just try to keep it calm and rational.

The only time I've ever talked to anyone about being overweight was a friend that lived out of state. I guess she thought I'd be one of those people a part of the fat acceptance movement and tried to unload some baggage on me about her weight problems. I chatted with her honestly and tried to be tactful about it but it ended up with her being upset and her not talking to me for most of the year. But almost a year later she messaged me that she had lost a bunch of weight with a new diet and exercise. I congratulated her and we chatted about other things but I interpret that last conversation as her way of saying thanks. So many people just avoid weight issues or beat around the bush until it's too late.
