The gearbox in the lower left has some gears in a vertical orientation and some in a horizonal orientation, to save space.
Here you can see how they had to shape the fork to save space, since the battery (currently removed) has to be big enough to accommodate the power draw.

Here's the movement from the back. The tuning fork curves all the way around the battery and is super thin. The nice thing about this movement is the fork frequency - it's a perfect 440hz, concert A natural. Quite a few musicians ended up wearing these to tune their instruments.
