Both the medieval pagan and nazi larp factions are equally far from the truth. You could visit a neofolk festival wearing either a medieval peasant outfit or a literal wehrmacht uniform including stahlhelm and people would probably smile at you a little bit but you would fit in just fine. Some concerts the medieval getup and some the wehrmacht uniform will be more appropriate. You will see multiple people in black suits and a bunch in elaborate 1920s/30s wardrobe in muted natural colours. Some bikers, some metalheads and many goths. Most people you will see wearing black cargos, Docs or milsurp boots and black shirts - somewhat martial, subtle gothic style. Some will make it folky by incorporating zunft jackets or trousers, some will make it more formal or intellectual. Most will incorporate one or two pieces of historic or contemporary European camouflage and nods to pagan religion such as runic, midgard serpent, kali or bone jewelery. In terms of hair style you will see undercuts, long hair and shaved scalps and many classic men's haircuts, often styled with pomade or hair oil. Females will approach all of this more playfully, you will see gothic latex princesses and cute milsurp goth girls decked out in different sorts of camouflage that you can really only see at neofolk concerts. You will also see people wear more experimental outfits oriented around religion and magic or history like pseudo-pagan religious garb in the style of Heilung, robes or literal fascist uniforms.

tl;dr actual real world neofolk style is subtle goth wear with martial influences (black shirt, black cargo pants, black military-style boots, undercut) often combined with European camouflage and nods to paganism (runic patches, sometimes bone jewelery) and modern reinterpretations of early 20th century intellegentsia/occultist wardrobe as well as some more high concept approaches built around the topics of magic, religion and European history.
