First, get into classic menswear on the whole and figure out how to get the fundamentals of fit, fabric, formality, function, and color right. Gentlemans Gazette is great for this. If you get it wrong, it'll throw off the whole look.
Specifically, look into Ivy style, as this is definitely the most Ivy League doctor look next to Matt Smith and Sylvester Mcoy

Once you've gotten that down alright, start searching eBay and etsy for vintage clothing in your size (GG will tell you how to find your size). make sure to check "Used".

Look for these specifically
>high waisted trousers
>waistcoats
>anything tweed
>anything corduroy
>knit button-up cardigans
>white dress shirts
>white wing collar dress shirts
>ties, ascots, neckercheifs,
>frock coats, 3/4 jackets, prince edward jackets, ex-hire, masonic, etc
>brogue boots, dress shoes, riding boots, cowboy boots, loafers, all brown ideally.

you can also find lapel pins, cufflinks, flared pants, and ascots on aliexpress for pretty cheap. don't use it for anything else though.

Don't compromise on quality for your main pieces, no synthetics if you can help it, certainly none in your shirts, plan things out so you can spend more on better quality items for a smaller but more coherent wardrobe.

Having one or multiple pinterest boards to use as a wishlist/outfit planner/inspiration boards is a good way of keeping track of everything
installing the pinterest web extension and saving images into a wishlist is better than using eBay's watchlist

as for the scarf, either ditch it or get something shorter and less obvious
