Pretty much what this guy said, I have 42 Geo's but they're roomy on me (how I like them). And a 41 in Ramone Socks. I'm tts 41.5-42.5 in everything from Rick to CP's to Margiela depending on the shoe as a reference.

Adding a strap might be tricky. I haven't looked deeply into the construction of the seam, but I don't believe its just attached to the lining, that wouldn't be secure and would cause the lining to bag and stretch. I think that it is attached to the lining seam and then anchored behind the seam to the actual exterior shell of the jacket arm seam. This might be a bit trickier (but not impossible) for a tailor to do, as it would require opening up the lining a bit.

But then again I might be completely wrong who know.
