You can use the same trick ebike motor makers sometimes use to get away with direct drive bike motors, ie. just make a large diameter motor. Gets you loads of torque at decent speeds without needing a transmission. You could easily fit a 30 mm diameter motor into a watch and suddenly 60 rpm for that motor doesn't sound like an impossibility after all.
