Just a galaxy s4, but you have to go into the settings on it and set the photos to a higher resolution than default for it to look decent. I set the phone on a little table I have in my room, and have a stapler keep it upright. The photos are also taken at a slight distance to remove a bit of the lens distortion. Then crop out my 10/10 model face and it's done. Setting the timer for five seconds, I take about two shots and that's usually good enough.
Also, I took a few test shots around my house to try to find the best lighting and angle.

That's a ton more than you probably wanted to know, but that's my process.
