I stayed in china in an apartment for a couple of weeks.
The "bathroom" was a toilet cubicle small even by western standards, which had a western toilet (good) however the shower for the bathroom was set above the toilet
so you could either sit on the toilet and havea shower or try and straddle the fucking thing
to top this off the whole "bathroom" cubicle seemed to be an after though to the apartment as it was set about a foot higher than the rest of it ( a literal step up)
this of course meat that while you're having a shower water was running out from under the door (it's just a normal door) into the apartment

Generally older homes have them like this. My house was built in 1863 and I have a toilet in my downstairs closet. My neighbors have one at the top of the steps like that, but they put up a little screen.

The reason they were put at the top of the steps is they were typically not upstairs during the day because it's fucking hot/cold in midwestern farm houses. Granted, some of those you posted are just weird.
