I just went to use my Sega genesis and the power adapter started making a high pitched whining sound.

It still works fine, but im a bit worried.

Whack it gently on the floor. Also, check what the output voltage and amps are so you know what to replace it with should it die later. You may have another around for another device that would work as a replacement.

Cheap transformers don't use a solid ferrite core, instead its some pieces glued together. When the glue wears down eddy currents can make the chunks of ferrite vibrate against each other. This shouldn't be dangerous, but it will reduce the maximum current it can provide.
