What's the process of commissioning a couch like? I feel like it would be virtually impossible to describe what kind of cushions and stuff you want without having it built by someone you know in person, are cushion specs even standardized?

I had a family member who used to do stuff like this for a living; not just furniture but all kinds of furnishings like lamps, curtains etc
They got most of their work from interior decorators in the region.
There are online services where you can commision custom furniture now, but I wouldn't have high hopes for that-- as you said, it seems like it would be hard to get a perfect match when you have very specific requirements.

Depends on what you mean by "cushion specs"...
Seating dimensions and a glass and such are standardized to some degree for mass production, but the fact that they are developed for an "average" person or for scale/aesthetic reasons not related to comfort is a big part of what drives the market for custom seating and other furnishings.

If you mean things like foam density and types of construction, suspension, etc. there's standard grades of materials and fairly standardized techniques for creating seating that's firmer/softer, better suited to lounging or task seating, made to stay visually crisp and straight vs going soft and looking lived-in, etc.
Best bet to get educated so you know what to ask for and what to avoid without getting pushed one way or another by a designer is to find someone in the upholstery supply business that serves designersxand upholsterers and pick their brain a bit; they see it all and can tell you what works and what to avoid for any particular application, and can also usually guide you to local designers and custom builders best suited for what you want, as long as you know generally what you want.

t. designed/built my own custom sofa with lots of help from the upholstery supply house, including saving me a ton of money by telling me to wait a month to buy foam to avoid a 600% temporary price spike because of a chemical plant going off line after a hurricane. Sofas 'R' Us & More Express Plus Factory Designer Outlet won't do that, not even during their Three Day Everything Must Go Going Out Of Business Labor Day Couch-Stravaganza Fire Sale they run six times a year.
