start with lost wax and something cheap, like hobby pewter or non-leaded tin/silver solder.
If you already have the machining experience for carving a steel or brass nut down to this level of precision, you wouldn't be asking here.

There are two parts to a ring like that - casting the body, then engraving the overall design and details. You can also buy blanks from jewlery supply to practice engraving on. Just google "blank signet ring".
Engraving, there's tons of books and youtube videos.
That specific ring, the guy who makes them, Ani Rings, probably has a mold for the block shape, and then refines it with engraving tools. He sells that design, so it's not a one-off.
So, for step one, either learn to cast silver, or have custom blanks cast for you (easy to find with google), step two, engrave.

A friend of mine started a custom jewelry company in his garage during the coof, he went from using blanks to learning to pour silver himself, and uses a 3D printer for the designs to make molds from. He's doing well with it.
