One can envision the differences of the write up by different parties....

Employee's reality:
Employee was scalded by boiling ink when fixing a printer jam. Extensive burns to the face and arms. Employee's synthetic shirt melted to his abdomen. Permanent disfigurement and diminish eyesight.

The business write up:
Defective printer cartridge fails catastrophically in the presence of unqualified employee breaking company policy. Employee was advised not to try to fix the printing machine in the past. Employee has been constantly combative to management. Employee has a history of work place aggression and it is likely he did this intentionally. Employee was promptly terminated after this blatant violation of company policy. Our company policy permits a single unpaid 15 minute break during an eight hour shift. We've deduced the employee was on his 15 minute break during his violation, therefore this is a non workplace injury. Therefore we also record no OSHA recordables. When the employee shrieked for help, we advised the employee he was no longer dressed according to company policy because he had black ink all over him and could not return to work. Promptly after this, when the employee's face was melting off, he cried for a first aid kit. Which we rendered for him from our company supplies.

Resolution:
Employee was charged for the destruction of company property and for the first aid kit we graciously allowed him to use. We are now seeking damages against the employee, as some of our other employees are traumatized by his act of self inflicted masochism.
