If I am at a nice restaurant I will do medium-rare.

Wagyu is the breed. That certification is for Kobe beef, which is a type of wagyu that comes from the Kobe prefecture and has all the standards/certifications you listed.

In the video, she even names the prefecture the beef comes from (not Kobe, forgot what it's called).

I prefer it medium rare to medium for the most part. Really depends on the cut and how it's cooked. Good quality steak medium rare.

I don't know, it makes no sense to eat RAW meat.

I'm really careful to not catch anything so I get mine medium well to well. I can see how steak is juicier the less it's cooked, but it's just not worth all the risks.
