Actually it's quite the norm in the whole country to serve spaghetti with these kind of clams (don't know if there's a short name for them in english, we call them "vongole" (VON-goh-leh), telline are very similiar but somehow more rare), and this is a pretty popular dish in seaside restaurants.
The clam is left inside its shell for mere decoration, when we're making spaghetti con le vongole at home i usually (and autistically) remove from its shell every single clam before i start eating.
