You either ordered a tough cut, or you were just eating not so great meat. Perhaps both.

If you're at a restaurant in the USA and the steak cost less than ~$30, you should ask them to use a jaccard tenderizer (assuming they're not doing it already). Really bad places do this by default, some good places do it anyway, but a lot of middling places don't because they think they're too good for it, so we end up with stories like yours.

If it actually was medium rare and not overdone, it was a bad steak. A good quality steak cooked a proper medium rare should be really tender. Did the menu say what cut the steak was, or give any other info about it?

Depends on the location. BTW, forgot to mention, there's also a fairly decent Brazilian all-you-can-eat "steakhouse" called Fogo de Chao. I enjoyed it the one time I ate there, but it's like any other AYCE place, you end up stuffing yourself and wondering if you really got your money's worth.
