Heat it to 110C for a couple minutes and you're very unlikely to get sick. Add water to counteract evaporation, microwave it, and stir once or twice during the heating, to distribute the heat.

Depends on the temperature where you left it. You probably wont get sick but the flavor might be a bit off.
I say chuck it.

I left some ching chong food out for like six hours a couple nights ago and it failed the smell and taste test, luckily it was just one dish and the rest was in the refridgerator. It still pissed me off that it was a waste when I'd have rather eaten it.

unless it was left out in the sun, it's probably fine. dairy doesn't spoil as quickly as meat, and it's probably got salty cheese in it which stops spoilage a little bit.
