A really good indie coffee shop requires a pretty loyal clientele (with the requisite disposable income) to support it. Not every town can sustain one.

Also if you like espresso drinks and are traveling Starbucks is going to be your only option in some situations, so you suck up your distaste for them and give them the business anyways.

It depends on what flavor you get. There are some that are actually cream based that I don't believe have any coffee at all in them and some with a coffee base. Unless you order an extra shot or 2 in one you won't be getting much caffeine even from the coffee based one's.
