We tend to use both phrases to refer to different consistencies of icing, though the specifics depend on the individual American. I would say that frosting is a whipped concoction usually found on top of cupcakes and such, while icing is more of a glaze (like on donuts). That's just me though.

Donuts have icing or glaze. Cookies and cakes can have icing or frosting.

Ganache? Ganache is made of cream and chocolate (and butter and/or salt, depending on the recipe). I would say it's more of an icing than a frosting.
