I have a grandmother who's kinda in that boat.
Lived by herself for several years, several states away so my family couldn't see her often.
Only family she had nearby was my uncle who (slightly understandeably) didn't want anything to do with her.
She never listens to doctors who tell her anything other than "all you need to do is take X", and she won't even follow the proper dosage most of the time.
She's also a diabetic and hell if I know which type but I know she doesn't follow her fucking sugar intake at all.

She used to OD on her sleep meds so much due to addiction that she started hallucinating about little girls in her room levitating and throwing pillows at her.
She complains about joint pain all the time when she just sleeps in all day.

The biggest thing though to me was her fucking old house. She was super obsessive over clinging to EVERYTHING that had any historic value, even fucking phonebooks.
The living room was dense as fuck with paper, and boxes were in the side rooms with random junk.
She had a three-room basement which were all completely full of various furniture and construction kits she just didn't get around to finishing.
The three major offenders though were her bedroom, bathroom, and kitchen. When we were cleaning her house I was cleaning out the fridge and found a plastic bag of black sludge that read "lettuce".
She LIVED there like that, for YEARS.

You know what her biggest problem was besides the joints and ODing on sleep medication?
She had a fucking salt deficiency once.
In motherfucking America.
And her favorite place to eat which she would eat every day if she could is fucking McDonalds.
That's IT, after living in that place for years and never listening to what her doctors said.
Also to put the cherry on top? She's fucking 80.
She's finally getting on track now that she's living near us and actually has family to talk to, but it is honestly a fucking miracle that she didn't fucking die.
