Assuming you're not deficient in any vitamins or minerals, maintaining a healthy weight is about the best thing you can do for your health. That said, a lifetime of McDonald's, even in sensible portions that don't exceed your energy expenditure and cause weight gain, is probably going to lead to poor health. Sort of ironic that a healthy weight person who only eats McDonald's is likely going to be healthier than a morbidly obese person who only eats lean meats, healthy fats, fruits and vegetables. Easiest way to get people to lose weight is to have them count their calories WITHOUT a calorie goal or immediate plans to change their diet. A lot of people don't realize they're over eating as a direct result of ingesting a calorie dense low volume food, especially if they drink the calories. If this man got a shake with any of his meals, even a small one, he'd be drinking more than two and a half burgers worth of calories in addition to the rest of what he ordered.

You just need to cut out the innocuous calorie bombs.

This is so true, so I'm the athlete from earlier that needs 4-5k calories a day but even then, I'm completely outclassed by the fat. One time I sat with a friend to help square away their lifestyle and get fit, the first thing we did was an honest assessment on dietary intake. On a normal day, they eat like 8k calories and perform no physical activity. On binge days, they can do like 10k to even 12k, how they don't go into toxic sugar shock is impressive in itself. The most difficult thing for them is to drop that dietary lifestyle, not the exercise commitment. In fact, I've seen that person try exercise without changing their dietary habits and now the exercise is triggering their hunger signals and resulted in them eating more than before trying to change their lifestyle. You're so close to understanding why CICO is pointless busywork. Literally just focus on only eating foods that fill you up as much as possible and you'll naturally eat less. Check out any given satiety index for an idea of the sort of things that fill you up the most per calorie. Plot twist, a lot of them are quite fatty, like meat. The tldr is eat more meat and veggies and you'll naturally eat less calories because you'll feel fuller.
