I think the original recipes called for tomato paste, water, sugar, and vinegar, so you can see why ketchup is now commonplace
Another common ingredient is mustard
Some people use barbecue sauce instead of ketchup, or the two mixed together

There is a variant called a "manwich" that adds red and green bell peppers in a fine dice

A third style is the "loose meat sandwich"
This only uses onion, sugar, salt, pepper, and mustard

All are cooked by mixing ground beef with water in a cold pan, then applying heat while stirring
This makes the ground meat come apart and not clump together

It's just ground beef usually served with this sort of can in the picture, you just pour the juice into the pan and mix it together. The point is service, or logistics if you will. Imagine your feeding 30, or 300 people. Making 30 burgers is a pita. Making 300 burgers is a serious operation.

My regional variation on these is the "tavern." So called, supposedly. Because it was popular in local bars and taverns. Since all you need to serve them is a warmer or crockpot, you can have a lunchon without a proper kitchen.

Usually in my house tavern sandwiches were prepared for nonstandard dinner situations like after football games or wrestling meets. The meat can be prepared in advanced and held in a cockpot so it's ready to go without immediate prep, whenever needed. Also common at family reunion potluck type events and community events. It's an easy cheap way to feed alot of people. Since the meat is cooked in its sauce it was typical for taverns to be served without any toppings. Or Maybe just pickles. So all one needed was the pot of meat and enough rolls. No condiments, sauces, cheese, etc.

I have fond childhood memories of summer days when all my cousins would come to town and we would have dinner at my grandma's house. Taverns and fresh sweetcorn. Self-serve typically.

My wife makes a pretty good mix that includes carrots and celery. Both add a slight crunchy texture after being cooked with the beef and sauce. I'm not too familiar with the recipe, but it's different from most recipes I've ever had growing up. It's the best as far as I'm aware of.

Depends on where you are.
In the midwest and most school cafeterias, they're mainly ground beef with a simple tomato sauce. But in the midwest you can also get "loose meat" sandwiches, where you sear the beef with onions then stir in Worcestershire and S&P.
At Sloppy Joe's in Key West (which claims to have invented it), you'll get ground beef "in a sweet, rich tomato sauce with onions, bell peppers, and spices."
If you make a Manwich at home out of a can, you get what others have described here, which is probably the most common American experience of the sloppy joe.
Or if you're not using the canned variety, a homemade version will often involve a tomato sauce with brown sugar, mustard, and Worcestershire, added to browned ground beef that has been cooked with onions and drained, then finely diced bell peppers mixed in at the end.
The more sauce-to-beef ratio, the sloppier (and cheaper) your joe.
