Probably uses the most high calorie options. Milkshake uses buttermilk or heavy cream instead of 2%, hamburger uses 70/30 lean/fat and so on. It doesn't even make sense when five guys burgers are so damn small compared to a home-made diner style burger. Maybe they just soak the shit in peanut oil.
