Pretty easily explained, it's hypovolemia that develops into a shock. The heart rate is a good indicator for this, it's a compensation mechanism. Blood pressure drops and the perfusion gets worse, leading to cyanosis in the lips (presumably fingers and toes too). Loss of fluid leads to the body redistributing. Blood thickens and that also makes perfusion worse. Young people often can't tolerate hypovolemia so excessive puking can lead to hypovolemic shock, which automatically goes into decompensated shock over time.
