South Koreans don't have the highest alcohol consumption but they consume the most liquor which probably damages their stomach. They also have a high rate of cigarette smoking which causes problems too. And they consume a lot of salt which is thought to remove a protective layer in your stomach that opens it up to bacterial infections which can eventually turn into cancer. Spicy food and fermented food probably don't play much of a role considering all the other factors, except maybe fermented food pushing their salt intake higher but eating a lot of other processed foods that have too much salt are probably more to blame for that too.

Korean frequency with stomach cancer is genetic, despite the spicy and salty cuisine. You'll find far more correlation with Koreans and alcohol with liver cirrhosis, ain't no shortage of older Korean men with severe liver issues. And like another anon commented, I don't think the cuisine is any saltier than any other national cuisine and you have no shortage of faggot Koreans who like to scold you for eating too "salty".

To answer your question anecdotally OP, Koreans definitely get that burning rectum when we eat janky spicy foods within our culture, such as packaged ramen or shitty street ddeokbukki (ricecake), or other variants of bbokeum (stir fried) spicy foods like bbul-dak (fire chicken which that shitty instant ramen is inspired by) or nakji-bokkeum (spicy octopus, frighteningly spicy in quite a few restaurants that serve it). If you're just eating kimchi and spiced fermented sides along with rice and soup/stews, the spice within them isn't abrasive and painful like a serrano or unlucky batch of jalapenos. Acid reflux and IBS is also something that is common yet not talked about among older Koreans as well.

I would've expected a stat much higher than that, however 25% isn't a big stretch. Our high intake of water from nearly mandatory soup dishes (albeit also commonly salty) at every meal and eating foods with lots of water content in general would probably cover some of that gap in sodium intake/exhaust. Fresh water availability has been historically generous in Korea and the cuisine reflects it, and nothing wrong with an extra (obviously not an exponential increase) bit of sodium as long as you collectively hydrate yourself accordingly.

Makes me wonder how much more sugar the average citizen of each nation consumes compared to others.

I mean if I chugged a big mug of beer or soda/water, that shit would slow down the absorption of salt anyway, no? Again in a previous comment that 25% increased intake isn't much of a stretch. No, I just meant that the water intake won't make a big difference if the water also has salt in it and it's being consumed with other salty foods too. Salt consumption in the USA is considered too high already but it's even higher in SK. I'm not saying it's the biggest factor in issues with stomach cancer, just one possible one along with everything else that's been mentioned, and obviously the risk for health problems with overconsumption will go up as more is consumed.

I want to say it's an age thing too, yet I'm jealous of my elderly mother and relatives who can still eat fiery hot without consequence while I'll be moaning and groaning on the toilet seat a few hours later. I imagine a huge proportion of us regardless of race or background suffer from the burning asshole if the spiciness is too much.

Greasiness and quality of the spice seems to be a huge factor for me too. Things like Tapatio, cheap/free red pepper flake packets, spicy instant ramen all destroy me. Yet things like Tabasco, good Mexican food, and most Korean food (except for ultra spicy dishes like fire chicken and octopus stir fry) leave my digestive system fine. I think everyone is truly a bit different.
