using stove top means, ie in a pan?

include toppings, condiments ect.

I cook mine on a flat top. Third pound choice, 20-25% fat. Salt, pepper, garlic powder (3-1-1), cooked medium rare. Toasted brioche. Pickles and tomatoes. After that it's a blank slate. I like arugula, but don't mind bibb/boston/butter. Onions can be white, red, grilled, caramelized, or green. I like smoked cheese (mozzarella, gouda, havarti), but don't mind pepper jack or emmental, or the occasional blue or goat; it really just depends on what goes with the other toppings.

1/3 pound, cook in cast iron, seasoning AFTER cooking with salt and pepper, medium rare. Remove patty add some butter, once melted use buns , pref kaiser, to mop up butter and meat drippings, do not toast yet, flip so buttered side is up. Put patty back into pan, add cheese on patty, pref sharp white cheddar, broil in oven till buns are toasted and cheese is melted.
Assemble as follows, arugula on bottom to protect bun from meat juices, patty with melted cheese, mustard, onions and tomato and finally the top bun.
Pref with a side of garlic fries.

Ah I'm retarded, only stove top means, then toast buns in pan and add cheese to patty the second it comes off the pan so the ambient heat melts it.
Broiler is more even toasting and nicer melt on cheese but if you don't/can't for whatever reason, still works on stove.

As fast as possible. Immediately throw buns in a toaster and press a thin patty into a pan at high heat. Salt and pepper whenever convenient. In five minutes flip it and add a melting cheese, in five more it's done. With practice and the right stuff in your fridge that's long enough to prepare a good number of toppings on the buns and put everything away. Then add the patty and clean the pan, finishing at 12-15 minutes.
A hamburger is a convenient lunch food that cries out to be endlessly modified and experimented with. Barbecue bacon yesterday, Oklahoma onion today, whatever's in the fridge tomorrow. You will find perfection only by making and eating many hamburgers.
