Have some tomatoes, white onions and chile serrano (if youre not a baby) cut in little pieces until it looks like pic related. Then add salt.

Anyways, there's a lot of ways to do it. Do it wathever way you feel like it. The idea is simple really, a easy made salsa of fresh vegetables without the need of a blender. Thats it.
