Honestly it goes to the whole stingy "customer is king" attitude we often have. If your not getting a loaded plate of food for the 14.99 your giving to the Applebee's than a lot of Americans feel like they are being cheated.
Its hilarious because most restaurants get around this by throwing a leaf of raw lettuce and a small ramekin of sauce on the actual plate being served and it makes them think they are getting their money worth.

This isn't really true, most American restaurants indeed do fill up the plate with carbs like bread or potatoes. Most American restaurants don't bypass the whole "we aren't getting our money's worth if our plate isn't full" issue with greens, most Americans get upset and feel like they are being cheated if they have too many leafy + raw greens on their plate because they know its not as calorie dense nor is it filling.

Basically, Americans do not think a salad on a plate "fills up" that plate because Americans are salad-aversive and tend to not count it towards a meal.

In parts of Europe (and also when I went to Korea and India) usually when restaurants give you a meal, they tend to fill up half or a third of the plate with fresh vegetables as well, but the main course itself is not that large and does not fill up the whole plate.
