Make sure your bread soaks in the custard for a while, don't just put it straight in the oven. Also make sure you have enough custard so that it doesn't get dry. I add almond extract and dried fruit both of which really take it to the next level imo.

I melt butter and brown sugar, then mix it with whipped eggs, milk/cream, cinnamon, nutmeg, clove and vanilla.
Then dump your bread in there, and put it in the oven after it soaked all your shit up.
I also like to top with a cinnamon streusel before baking. Maybe make up a brown sugar rum sauce for some topping if people really want to get decadent.

baking it without a water bath

you just cube up bread, stale works fine, soak it in milk, egg, sugar, opt. cinnamon/nutmeg/cloves

and bake but make sure to put the (greased) pan with the bread pudding in it in a larger pan filled with an inch or two of hot tap water and cook until the center reads about 160F on a thermometer or a skewer/toothpick comes out of the center with a few moist crumbs but no longer wet.
