I don't think they're meant to be a refined thing per se. Cigars are just tobacco leaf wrapped together and a lot of the mystique and luxury aspects are negated when you remove more recent taxation and realise that you can get a decent cigar for cheaper than a pint of beer and it'll likely last longer. Sure, there's always also been the upper end but cigars, like pipes, have previously been a universal form of smoking and the idea that you need to be dressed in a dinner jacket and lounging in a furnished smoking room to have a cigar is pretty silly. That said some people do think that all they need to do to look sophisticated is smoke cigars and I've seen people admit that they're trying to look distinguished while dressed in cargo shorts, flame t-shirts and flipflops. Cigars and pipes have a much lower risk of cancer than cigarettes because you don't breath the smoke.
I make the comparison to chewing tobacco because like chewing tobacco you are getting the nicotine hit entirely through the mouth.
The biggest risk of cancer for pipes and cigars is from resting them directly on your tongue, which you're not supposed to do. Drinking alcohol with them exacerbates this.
