Can a jew tell me if this is actually true or if it's just ultraconservative orthodox rabbi stuff and no jew nowadays follows this? Why do you leave out the best parts of the cow? What do you do with it?

Reform Jew here. Only conservative or old fashioned people really follow the kosher rules. I think my rabbi does, but she does it more for the street cred than any conviction.

I eat some kosher stuff, but mostly for the flavour and the homey vibe it gives me. Same reason I eat halal foods.

Hemisemidemi Jew here. By which I mean I'm Jewish, but not the sort of Jew you're probably thinking of. I'll answer from the perspective of our traditions and, to the best of my knowledge, that of mainstream Ashkenazi Judaism's.

Yeah, it's true...ish.
Remember when... Jacob, I think? — was fighting the angel and the angel literally beat his ass? G-d transferred Jake's pain to some nearby cattle, so the meat afflicted by this pain is considered terifeh IE as having been mauled. Meat that's been mauled is non kosher.
However, this is mostly a concern for Ashkenazim. Maybe Sephardic Jews, too but idk for sure. I'm a different type with our own halakhic traditions. We even pronounce words differently ('terifeh' from above is pronounced as 'try-fuh' by Ashk Jews).
In any case, even Ashks can eat this back end, but it requires, no joke, surgery. Basically, the slaughter house must employ a surgical veterinarian or, at the very least, a super fucking skilled butcher, to remove the sciatic nerve without severing it.
We make fun of Ashks because they keep finding ways to circumvent the Bible. We say that if you're looking for loopholes, you should probably research another religious tradition and convert to that cuz y'ain't followin' this one in good faith, homie.
