Cameras focus at a certain distance, auto-focus tries to figure out what the best distance should be, usually by just getting the greatest area of the screen in focus. Putting your hand behind the object (especially small objets) creates a greater surface area closer to the lens to force the camera to adjust its focal distance.

Smart technology dude. It's looking for faces, and autofocuses farther away from the new item held up. So temporarily putting something else new, block the other centered item, the room/the face, causes the camera(s) smart technology to pick the close item to focus upon. It lets people see something close-up such as in a live video, or to prevent some video editing later when they focus upon it and splice that in.
