"Spaghetti" as a dish, the way that it's prepared by flyovers in a sugary tomato sauce with meat, does not exist in Italy. Spaghetti as a shape of pasta exists, but it's really not any more common than other shapes. Pasta dishes in Italy are named along the lines of "X pasta in Y sauce" e.g. spaghetti alla carbonara.
