Get some good quality white bread, not the cheap stuff. A country white loaf from a local bakery is ideal. Get good cheese too, not Kraft singles. I personally like 3 slices of sharp cheddar, but provolone or pepper jack is also quite good. Build the sandwich, then spread room-temperature butter on the outsides. Put it into a hot panini press and cook until moderately browned.

In my experience, for sandwiches, Americans pretty much always buy pre-scliced cheese whether it's crappy american "singles", pre-packed cheese slices or better quality cheese sliced at the deli counter. Especially with grilled cheese, it's meant to be a cheap kid's sandwich made from white bread and American cheese. It's nostalgic for most Americans, and they like to eat it how they grew up with it.

Every American home has at least one simple cheese grater, but what cheese they use for everything else varies. The deeper south and flyover a suburban household is, the more likely it is they use pre-packaged, pre-sliced, and pre-shredded cheese - again, based on my experience. I wouldn't be able to tell you about rural households, and in cities there are many different cultures and wealth levels to account for.

I have never heard of an American household without a kettle, so I'm not sure where you got that from. Tea is nowhere near as popular here as it is in Europe, and coffee makers are common, so I can see how a kettle wouldn't be as necessary for some Americans. But it's still something I know everyone to have. Plus, sweet tea is a huge deal throughout the entire south and possibly midwest. Even the folks I know who think Applebees is a fancy night out like to brew their own sweet tea.
