its pretty simple, it represent a phase of the pasta cooking, if you use barilla the package most straightforwardly gives you a recommended cooking time (assuming you are not under sea level or over 1000 meters in altitude it should work just fine) if you cook the pasta in boiling water only you will get a normal cooked pasta in the given time
take it out one or two minutes earlier and you will.get firmer al dente pasta, which not only will taste delicius but will be easier to digest, and stay compact much more time it you want to do pasta salad

about cooking, elders here are ADAMANT about overcooked soggy pasta, they think its easier to digest which is half true for the stomach but totally wring for the intestine and colon, I cook my pasta barilla number 72 for three and a half minute in boiling water, plus 30 seconds to handle the pot, the result is extremely firm, you will get quite a.mouthfull to chew but your bowel will thank you

I do this because they're so used to it you'll get it overdone if you ask for medium, but still actually rare if you ask for rare. I actually prefer just shy of medium so it usually works out, and if it's actually medium rare, that's my second favorite, so just dandy.
The real issue is over easy/over medium. I hate uncooked whites, it completely ruins the eggs. But most people order over easy when they mean over medium, and over medium ends up served overcooked as a result, so I order over easy and pray they do it with the whites cooked.
