That's just what the words mean. If you order a big mac it comes with big mac sauce; that's part of what makes it a big mac. But it's up to you as to whether or not you dip your fries in ketchup.

Furthermore, if you ask for a topping to be served on the side but then apply it yourself at the table it's still a topping, unless you dip into it or spread it on with each bite, in which case it becomes a condiment.
