Why are they so inconsistent? Sometimes they're good sometimes their bad most of the times they're mediocre. What are the most consistent items on the menu that you can get that are good 100 percent of the time because every time I go it seems like I am playing Russian Roulette.

It's inconsistent in the sense that every thing is precooked, and sits in a plastic tub in a baimorie for X amount of time. You might get lucky with freshly cooked, or else you might get something which has been sitting its own grease and the grease of other burgers for hours.

If you're picky just ask for fresh and it will be more consistent, you'll have to wait mildly longer.

Like all widespread services it depends entirely on the employees that work there. There's two macdonalds at opposite ends if town here, and they have very different staff members.
One is employed by middle aged ladies who have been working there for a long time and know how to get around the kitchen and make the food in perfect time. The other uses college/high school kids that constantly change lineup with the seasons and are only there to make some spending cash. Guess which one always fucks up my fries and more likely to spit in my drink?
