I'm not sure what the issue is with the people you are describing who collect hot sauce made from x chilies or think the latest ghost pepper is a good dare, and like the burn over flavor element more than the flavor element of chilies. Sauces are a way of preserving a harvest of chilies, so in a way they're a fermented craft food if done right, and that's commendable. I've had some very excellent homemade hot sauces in lebanese restaurants over the years, and some very good harissa a libyan boss used to bring to the office, that was predominantly fresh garlic, olive oil and some unknown chili from his garden.
For the hotheads, It is a built up tolerance for sure. If I want hot food, I just cook with fresh chilies which I deseed so I can include more chili flavor before I can't tolerate the heat, or tuck a little half teaspoon of chinese chili paste from the jar into the dish. Or make my own chili oil by simmering some dried pasilla chilies in oil til crispy. I like to take the crisped chilies and oil and pour over popcorn, too. I love chili flavor, but not burning my mouth out. If I'm using scotch bonnets, it is for jerk pork to put on the BBQ and I'm taking those seeds out. I very rarely use bottled sauce on the table. I see its point on the table at breakfast where people might want a couple drops of cholula or Crystal on an egg sandwich. In Mexico, they'd have a fresh salsa on the table at breakfast anyway.
Bot "hotheads" as you describe them go beyond the enjoyment of a enjoying an indian or thai meal with chilies, delicious burn with lots of other enjoyment involved. Sipping a Tsingha between bites, or a cold iced coffee with the cooling sweetened condensed milk. It's not that experience at all, it's adding so much hot sauce that vinegar is surely ruining the balance.
